Our Objective

  • To educate & reinforce the importance of following basic medication guidelines in the home environment

  • To improve compliance of Hospital / Doctor Discharge Plan

  • To reduce unnecessary emergency room visits, hospitalizations & doctor visits

 

  

First Visit

  • First Visit will consist of a 30 minute interview between Pharmacist and Primary Caregiver(s)

  • Follow-up to be done 30 days after initial visit

  • Copy of Medication Profile will be provided to Primary Caregivers(s)

  • Our Club Members will be able to access our Pharmacist for any Emergencies 24/7
